A prisoner at HMP Oakwood has died aged 75, a notice from the Prisons and Probation Ombudsman has confirmed.
Graham Chambers died on 31 August 2026 at HMP Oakwood in Wolverhampton.
The privately run men’s prison in Featherstone, Wolverhampton is managed by controversial security company G4S.
No cause of death or details about the circumstances are provided.
The Prisons and Probation Ombudsman lists its investigation into Chambers’ death as ongoing.
