An independent investigation has been published into the death of a 57-year-old prisoner at HMP/YOI Parc in Bridgend.
Peter Kaminsky died from metastatic pancreatic cancer on 4 October 2025 while he was a prisoner at HMP Parc.
According to the Prisons and Probation Ombudsman (PPO), Mr Kaminsky had been sentenced on 21 February 2025 to 54 months in prison for arson.
Following his death, the Ombudsman launched an independent investigation into the circumstances surrounding his care and treatment while in custody.
Healthcare Inspectorate Wales commissioned an independent clinical reviewer to examine the clinical care Mr Kaminsky received at the prison.
The review concluded that the clinical care provided to him was equivalent to what he could have expected to receive in the community.
It also found that healthcare staff at HMP Parc acted promptly to seek urgent medical attention when this was required.
The PPO separately examined non-clinical issues relating to Mr Kaminsky’s care and reported that it did not identify any non-clinical issues of concern.
The initial investigation report was shared with His Majesty’s Prison and Probation Service and Cwm Taf Morgannwg University Health Board.
According to the Ombudsman, neither organisation identified any factual inaccuracies in the report.
Mr Kaminsky’s family was also provided with a copy of the draft report and did not make any comments.
The PPO also informed HM Coroner for South Wales Central about its investigation.
The report states that the Coroner confirmed they would not be holding an inquest, and a copy of the Ombudsman’s report was provided to the Coroner.
The investigation was signed by Prisons and Probation Ombudsman Adrian Usher and dated May 2026.
The findings relate specifically to the circumstances of Peter Kaminsky’s death and the care he received while at HMP Parc.
