Operation 0 is our mission to bring the number of preventable deaths in UK prisons to zero.
Every life lost is a failure of the system, and we are determined to demand accountability, transparency, and reform to ensure no more families endure this pain.
Since February 2022, 31+ lives have been lost at HMP Parc alone. Across the UK, this number continues to rise due to neglect, poor conditions, and systemic failures.

Since its creation, HMP Prisons’ Justice Group has uncovered shocking levels of corruption within HMP Parc and beyond. Families impacted by the tragic loss of their loved ones have bravely spoken out, with multiple stories featured in the media, bringing national attention to this growing crisis.
Our investigations have revealed systemic failures, mismanagement, and a lack of accountability, all of which have contributed to the preventable loss of lives. These stories are proof that change is not only necessary, it’s overdue.

Hold G4S and those responsible at HMP Parc accountable for the 31+ preventable deaths since December 2022. - Justice for Families
Support families seeking answers and ensure they receive the justice they deserve. - Stripping G4S of Management
Advocate for G4S to be removed from its position of power and for systemic reforms to replace failing private management structures. - National Awareness of Systemic Failures
Highlight the wider issues of corruption, neglect, and systemic failures within UK prisons. - Zero Preventable Deaths
Push for policy changes and reforms to create a system where preventable deaths in custody are no longer a reality.
